FDE's Free Diabetes Supplies
Warehouse®
To start, please
browse thru the list below of all the items that are available to
anyone who is eligible to shop in our Free Diabetes Supply
Warehouse®.
Make note of the items that you have or are now using that you
believe are / were working for you, as well as the items that you
would like to try
that you feel your doctor will approve by clicking in the box in
front of the Product #.
If you want more information about the product, click on the
product# itself.
The data that
you provide on the Eligibility Form will enable us to look for insurance, private,
pharmaceutical, or government programs that may be of assistance.
Then, within 3 business days, we will email you notification of the
programs, if any, that you qualify for along with the
appropriate forms for you to complete.
These forms usually can be faxed to your doctor for signature and
their return to the appropriate program administrator. Once
approved, you will either receive the supplies via the doctor’s
office, by mail, or through a local pharmacy.
This service
is absolutely free. Likewise,
there is no charge for insulin or any other prescription drugs for those who meet the eligibility
requirements! And, for all California
residents,
all the items (including test strips!) sent
from the FDE Free Diabetes Supplies Warehouse®
are absolutely free, as well – the FDE even pays for their
delivery!
